Hamed from Egypt made a historical victory over the international competition and won Frederick Banting Research Award-2015 in Diabetes. Their research was highlighted in international media and gathered a large worldwide attention in scientific circles while it appeared in world's most read journal 'Journal of Diabetes and Health'. In their report on Evaluation of some urinary biomarkers for predicting early nephropathy in type 1diabetic patients they mention that Objectives: The purpose of this study was to define a panel of proteins that can serve as novel biomarkers of nephropathy in type 1 diabetes. Material and Methods: The study included 60 type 1 diabetic patients and 20 healthy subjects, All groups will be subjected to: FBG, HbA1c, Creatinine, Urea, Microalbumin, creatinine in urine, lipid profile, THP, Progranulin, Clusterin, AGPandProst D levels were determined ACR and GFR were estimated. Results: There is a significant decrease in THP in the macroalbuminuria group , the microalbuminuria group and the normoalbuminuria group in compared to control subjects, also there is a significant increase in progranulin in the macroalbuminuria group , the microalbuminuria group and the normoalbuminuria group in compared to control subjects, also there is a significant increase in Clusterin in the macroalbuminuria group and the microalbuminuria group in compared to control subjects and the normoalbuminuria group also there is an increase in AGPandProst D the macroalbuminuria group , the microalbuminuria group and the normoalbuminuria group in compared to control subjects. Conclusion: THP, Progranulin, Clusterin, AGPandProst D level in urine may serve as early predictors for nephropathy in type 1 DM. Under their leadership the research area is surely likely to expand beyond the horizons. It will be worthy to explore the further potential under their legacy. Ibrahim A. Emara, Mohamed H. Authors concluded that this panel of urinary proteins can serve as new tools for the prediction of diabetic nephropathy in early stage in type 1 diabetic patients.
From the present study urinary THP, Progranulin, Clusterin, AGP and Prost D are most important powerful new tools for diagnosis and surveillance of nephropathy in type 1 diabetic patients. Also the current study provided important data on a panel of proteins that could be used to predict the early signs of diabetic nephropathy including the development of micro and macroalbuminuria as well as significant renal function decline including the ACR and GFR. By using these biomarkers as novel markers of diabetic nephropathy this will be minimize the disease progression in the diabetic patients.
The important limitations in this study that done in a small number of populations of patients.
Further studies of this protein panel is needed to verify their predictive ability for the development of both renal function decline and urinary albumin and to determine if these could be used as biomarkers of nephropathy and disease progression. Further study must be done in large scale and other population of this panel of proteins to reach the cut off points of each protein in the diagnosis of nephropathy in the early stage.
