Adegun Patrick Temi, Omonisi Abidemi Emmanuel, Atiba Adeniran Samuel, Areo Peter Olufemi, Ajayi Oladimeji Akande, Taiwo Olufemi Joshua, Esho Julius Olusanmi and Odesanmi William Olufemi from Nigeria made a historical victory over the international competition and won Max Planck Research Award-2015 in Urology. Their research was highlighted in international media and gathered a large worldwide attention in scientific circles while it appeared in world's most read journal 'The Journal for Urology'. In their report on Prevalence of Clinico-histologically diagnosed Prostatic Diseases: Experience from a budding Tertiary Centre they mention that Introduction: Prostatic diseases have emerged as significant health challenges in most elderly man aged 50 and above. There is no existing literature on the prevalence of prostatic diseases in this environment. Recently, there has been renewed interest in understanding the wide range of diseases affecting the prostate glands. This retrospective study was to describe the prevalence, clinical features, histology and pathological characteristics of prostatic diseases in Ado-Ekiti, Nigeria. Materials and Methods: The results of all prostatic tissue specimens received in the Anatomic Pathology at Ekiti State University Teaching Hospital, Ado-Ekiti from January 2008 to December 2013 were analyzed in addition to the patients’ clinical details. A total of 141 specimens were studied. Gleason’s microscopic and the TNM classifications were used to grade all the malignant lesions.PSA was correlated with the malignant cases. Results: The minimum age of diagnosis was 45 years while the maximum age at diagnosis was 95 years. The mean age for BPH was 72.6 years; mean age for adenocarcinoma was 74.4 years. History of poor urinary stream in was the commonest symptoms. There were strong associations between adenocarcinoma of the prostate with weight loss and back pain [p<0.05]. Significant numbers of the prostatic adenocarcinoma showed abysmal elevation in the levels of PSA with a mean value of 66.2 ng/ml. Conclusions: This study has shown that prostatic diseases have emerged as a major health problem in Nigerian young adult males aged 45 and above and no longer the disease of the elderly only. The level of serum PSA for subjecting patient to prostatic biopsy must be decided alongside the clinical details and not just based on the recommended cut-off points in the literature because of the abysmally high levels recorded for BPH in this environment. Under their leadership the research area is surely likely to expand beyond the horizons. It will be worthy to explore the further potential under their legacy. In conclusion, this study has shown that prostatic diseases have emerged as a major health problem in Nigerian young adult males aged 45 and above. It’s therefore necessary to differentiate the various entities of prostatic diseases using the international approved clinico-pathological criteria.
BPH constitutes vast majority of these prostatic disorders and this lesion has shown histologically, the presence of accompanied lymphocytic aggregates and corpora amylacea in most of the biopsies examined. The Prevalence of Pca is rising in this environment and most of the cancers are quite aggressive with a Gleason’s score of 8-10 and may be fast-growing. The prevalence of advanced Pca in the age group 41-60 in this environment is worrisome and requires further research.
The level of PSA for making a diagnosis may need to be reviewed upwards because of higher levels recorded for BPH.
There is an increase rise in the prevalence of BPH and Pca in the young adults in this study. The serum PSA levels of BPH in this study are higher than those of the Caucasians. Poorly differentiated adenocarcinoma is quite rampant amongst patients with Pca.
Being a retrospective study; it is difficult to control the collection of samples and their interpretation. Some of the patients who cannot access the tertiary health centre may have been managed in a private hospital and therefore not captured in this study. This will affect the overall prevalence in this environment.
Awareness campaign on the rising prevalence of Pca should be vigorously pursued in this environment so as to stem the trend of advanced stage presentation. Screening for Pca in men with average risk may be commenced at age lower than 55 rather than the no screening for age 40-54 recommended by AUA in this environment. Any male patients above the age of 40 years complaining of back pain and weight loss with average or higher risk, screening for Pca among his screening profile may be necessary. Government must create a policy that compel men aged 40 and above to go for PSA screening once in a year.
